NEW PATIENT PAPER WORK
New Patient Paperwork must be completed and turned-in to the office prior to scheduling a first appointment; you may mail or drop off the forms.
This is an outpatient practice that only provides coverage during the time the office is open. Anyone whose condition requires 24-hour, 7 days a week monitoring, back-up, or admission to a psychiatric hospital should find another practice that offers these services.
